Laboratories may subscribe to one or more tests by following the link below and adding tests to the shopping cart. If payment is made via credit card, continue through the checkout process and select the credit card payment option.

If payment is to be made via purchase order or if your agency would like to be invoiced, please complete the order using the same link but write your PO number in the "Special Instructions" section of the checkout and use the "print and call" option when submitting the order. Print your receipt and instead of calling the number given, please FAX the receipt to 517-579-4847. An invoice will be e-mailed to the purchasing agency.
You may also use the following form to place your order by fax. Please attach purchase order to the fax or write your purchase order number on the fax cover page.
